Provide medical records that demonstrate the qualifying condition, OR pay $29 Diagnosis Fee

Meet with our Healthcare Practitioner at your telemedicine appointment

Once approved, the Health Care Practitioner Certification (HCPC) will email it directly to the patient to then be submitted to the state
