Complete new patient forms
Step 2
Attend your appointment

Appointment typically takes less than 15 minutes

The provider will review the medical records submitted that demonstrate a qualifying condition

If medical records not submitted, a diagnostic medical exam can be completed at the time of the appointment for $29 fee

Once approved, the Health Care Practitioner Certification (HCPC) will be emailed immediately to the patient

Register with the State

Submit a Digital Photo State ID or Driver's License

Upload the completed Health Care Practitioner Certification form (HCPC)
